| Unlock member rewards with timely claims submission | | As of May 1, 2026, our Healthy Rewards program for members has transitioned to a claims‑based model for most rewards. Meridian members must complete appointments or health activities they qualify for, and claims must be submitted to Meridian before incentives can be redeemed. Timely submission of claims is critical to ensure your Meridian patients are rewarded promptly for preventive care. Checkups for babies, children, and adults will continue to be attestation‑based for the program. | | | |
